are now being accepted for this event. The cost per
four-member team is $200. To register, list the
names of team members below and mail this entry form
along with the entry fee to the Danville Police
Department.

Please note: individual
players may register for a fee of $50 each. Every
attempt will be made to assign individuals to teams,
but there are no guarantees. Money will be refunded
to individuals not assigned to a team.
